Assertive Community Treatment (ACT) is an evidence-based practice that improves outcomes for people with major mental illness who are at greatest risk of psychiatric crisis, hospitalization, and involvement in the criminal court system. ACT is one of the oldest and most widely researched evidence-based practices in behavioral healthcare for people with severe mental illness.

ACT is a team of professionals working to assert outreach in the community. Quality of life is reached through consistent, caring, person-centered relationships. Research shows that ACT reduces hospitalization, increases housing stability, and brings positive outcomes for people with the most severe symptoms of mental illness.
